Fife Health and Social Care Partnership, in conjunction with Fife Carers Centre, are launching the Multi-Agency Guidance for Transition Planning for Children and Young People with Additional/Complex Support Needs.
The event will feature a presentation from Hannah Weale, Lead Officer for Children and Young People with Additional/Complex Support Needs, outlining the revised transitions procedures. Updated communication documents for young people and families/carers will also be shared, along with the opportunity to ask questions.
